I understand that any false statement contained herein or omission of any fact may result in rejection of this application or termination of my employment if such false statement is discovered subsequent to employment. I authorize the company to request any persons, firms or corporation identified as a former employer or reference to answer any and all questions that may be asked and to these persons to give any and all information concerning me, my work habits, character, or skills that may be sought in connection with this application. I expressly release these persons from any and all liability in furnishing responses to these inquiries.

I AUTHORIZE FOREMOST PAVING, INC. TO OBTAIN A CONSUMER REPORT, WHICH MAY INCLUDE, BUT NOT BE LIMITED TO, A CREDIT REPORT AND/OR A CRIMINAL BACKGROUND CHECK, ON ME FROM A CONSUMER REPORTING AGENCY AS PROVIDED FOR AND DEFINED BY THE FAIR CREDIT REPORTING ACT. I UNDERSTAND AND AGREE THAT A CONSUMER REPORT MAY INCLUDE, BUT IS NOT LIMITED TO ANY WRITTEN, ORAL OR OTHER COMMUNICATION OF ANY INFORMATION BEARING ON MY CREDIT WORTHINESS, CREDIT STANDING, CREDIT CAPACITY, CHARACTER, GENERAL REPUTATION, PERSONAL CHARACTERISTICS OR MODE OF LIVING.

I also agree to submit to medical examinations as required by the Company and as may be permitted by law. Additionally, I will submit to drug and alcohol testing as required by the Company and appropriate government regulatory agencies. I will also engage in necessary activities to demonstrate my ability to perform the position sought through trial, student trips and other Company designated activity.

I understand and agree that, if employed, my employment is for no definite period and that I may be terminated at any time without any prior notice, regardless of the date of payment of my wages or salary. If this application is considered favorably, I agree to abide and comply with all rules and regulations of Foremost Paving, Inc. as well as they currently exist and/or as they are modified from time to time during the employment relationship. I will abide by the safety rules of this Company. If injured, I authorize my employer to use best judgment for treatment unless I instruct otherwise.
